Care Coordination
Care Coordination provides a community based healthcare solution based on
partnerships with clients and families, healthcare professionals, support
workers and community agencies.
There is one Care Coordination Centre in Christchurch and two based in
Wellington.

- Capital & Coast Care Coordination
- Hutt Valley Service Coordination Centre
Each service works to identify the priorities and goals of clients, their
families, and providers, to achieve the best possible level of independence.
Some of the services covered by Care Coordination include:
- community allied health services
- community rehabilitation services
- district nursing services
- home based services, including home help and personal care assistance
- access and entry to residential care
- respite care
- day care
The Care Coordination Centres are staffed seven days a week.
Hours of service:
8am-6pm Monday to Friday
8am-4pm Weekends/Holidays
There is also an after hours service for urgent calls and referrals.
