Following the inaugural Case Management Conference held in Taranaki in 2007,
there was keen interest to better understand and define Case and Care
Management. With a rapidly growing older population, and a need to better
support people with chronic conditions and disabilities in the community, focus
is being placed on roles that can successfully achieve the desired goal of
ageing in place. Building the correct roles within the right models of care will
strongly influence the successful support and management of older people in the
future.
While much is already understood about Case/Care Management, there is also a
lot that is still unclear and confusing. Roles are not recognised within a
competency framework in New Zealand or supported by a professional body and
practice standards. If these roles are one of the potential solutions for New
Zealand in the management and coordination of community care, then it is time to
explore these issues and move forward.
